School Health, has updated the application webform to reflect Colorado's eligibility. Please feel free to visit https://www.schoolhealth.com/neffy to submit your application at any time when you are ready to do so. 

 

You will be prompted to provide medical authorization for neffy.

A medical authorization template has been attached if you need one.
