Reporting Guidelines for Accident and Incident Reports
When?:

Lacerations (cuts) that might require stitches (usually ½ inch or more)

Broken bones (real or suspect)

Severe swelling/bruising of ankle, knee, wrist, fingers, etc.

Significant head injuries (black eye, memory loss, confusion, loss of consciousness (LOC)

Injury or sudden illness that requires the parent/guardian to take the student directly to the physician from school (ER / Urgent Care / Dr. Office).

Anytime you call 911
Accident/Incident Report:
-
Top portion completed by staff/person involved with incident
-
Lower portion completed by person providing care in health room
-
Original copy in health file
-
One copy to Superintendent
-
One copy to District Nurse
Infinite Campus:
-
Enter information in Health Office Visit - be concise and give facts as well as a clear timeline of events

Enter appropriate discharge (911-ER, urgent care, etc.)

Also enter a second discharge for "Incident Report in file/email sent." This allows for incident report data collection from Infinite Campus
Email (every time incident report completed - no copies need to be sent):
Put Student # ____ Incident at (Name Of School) in subject line. Give a brief narrative of events and how/when the student was discharged.
Telephone Report (only when incident is life threatening):

Call to Superintendent

Call to District Nurse
Head Injury Forms:

Completed by health room assistant, including details from person who witnessed event
-
ALL head bumps, regardless of apparent significance
-
Call parents on all head bumps and send a copy of the form home

Keep original in the health file.

Additional "Incident Form" necessary only if injury is severe (see above)
-
Email or call to the Superintendent and District Nurse made only if severe enough to warrant an incident report.
